Select “Custom Forms” from the left side Menu and Click on “Forms” button
On “Custom Forms” page, when you click on “+Add New Form”, the “Form” page pops up
On the Form page, enter Form Name
Click on the radio button to choose the type of form – Product Related
Select Header Icon and Product Icon from the list and choose the group and users
Choose the categories to show the form and click on "Save" button
Now, on “Custom Forms” page, click on more icon at the right side of name field
Now, you can see “Add new customer field” and “Add New Product field” section as shown in the image above
Choose a customer field and click on pencil icon to edit the customer details
Choose the field type, enter the field name, and click on "Save" button
On the “Custom Forms” page, select a field of product field and click on pencil icon to edit product details
Now, you are on the dialog box for mapping the product image with a category
Now, click on the radio button of “Product Image”
When you see the category turn from red to grey as shown in the image above, click on "Save" button
On web catalog, users can see the image of the pick list items on “Order Cart”
